We are grateful to Dr. Chan and his colleagues for presenting an interesting case with innovative surgical techniques, in response to our published article(1). We reported an anatomical pitfall in right posterior sector graft (RPSG) procurement in living donor liver transplantation (LDLT). Functional graft volume in an RPSG should be evaluated based on not only the right hepatic vein (RHV) and inferior vena cava (IVC) but also portal perfusion. Chan et al. successfully modified the RPSG without RHV by reconnecting V6/V7 tributaries and IVC with a cryopreserved vein graft. Their meticulous way of reconstruction is very creative and impressive. We have several points to discuss regarding their graft selection and surgical techniques.
