低血糖を主訴に紹介された乳児ボツリヌス症の1例

症例は6ヵ月の女児。入院前日から活気低下と哺乳力低下があり、ケトン性低血糖が認められた。輸液により低血糖が補正されたにもかかわらず口渇と発汗低下を認め、急速に筋緊張低下が進行し、無呼吸、瞳孔散大などを認めた。その後、血清および便中からB型ボツリヌス毒素が検出され、乳児ボツリヌス症と診断した。非侵襲的陽圧換気による呼吸管理と経管栄養を行い、約1ヵ月半の経過で改善した。稀な感染症であるが、便秘、口渇や発汗低下のような自律神経症状を伴う乳児の急性弛緩性麻痺では乳児ボツリヌス症を鑑別に挙げる必要が...
