本研究の目的は,ケアの受け手である妊産婦を対象に,私たち助産師に対する認知状況や役割期待を明らかにし,課題を把握することである.自記式質問紙調査票を用い,産後1か月健診を受診した褥婦959名を対象とした実態調査を行った.その結果378名(回収率39.4%)の回答を得,以下3点の課題が見いだされた.1.岩手県の地域特性については,義父母または実父母と同居している者は約3割ほどであるものの,親世代が同じ市町村,近隣の市町村など距離的に近い場所に居住して者の割合が高かった.通院に関しては,9割以上の者が自家用車を利用していた.所要時間の平均は34.1分であり,病院選択時に困惑した経験を持つ者は95名(25.1%)であった.2.助産師の認知状況については,90%以上の者が認知していた.しかし,「自分が出産した病院に『助産師』がいた」と回答した者に対し,その理由をたずねた結果では,「助産師から名乗りを受けた」と直接的に認知した者は3割にも満たず,多くは「名札をみた」,「掲示板をみた」など間接的な認知状況であった.業務内容の認知については「出産」,「産後の母子のケア」についての認知が高く,妊娠期～産褥期・新生児期に各時期における健康診断に関する項目の認知が低かった.3.助産師の役割期待として,妊娠期では「超音波写真など思い出品がほしい」,「児の世話についての指導」,「不安や訴えをゆっくり聞いてほしい」など,今後の助産師外来開設の広がりにより改善できる項目が多かった.分娩時には,「好きな姿勢で出産したい」,「自宅分娩をしたい」といった主体的な出産に対するケア内容よりも「いきみののがし方を教えてほしい」,「そばに付き添っていてほしい」といったスタンダードなケアを求める声が多かった.また産褥期には「児の健康状態の判断方法の説明」,「児の健診や予防接種に関する説明」,「母乳栄養支援」など退院後の生活を見通した支援内容の期待が高かった.A study was conducted to clarify the qualities recognized, and those expected, of a midwife using a questionnaire distributed to 959 women after childbirth in Iwate Prefecture. Responses were obtained from 378 of these women, and the data obtained revealed the following. 1. About 30% of the surveyed women were cohabiting with their parents, or living within a short distance from them. At least 90% of the women went regularly to hospital by car, taking an average of 34.1 minutes. Ninety-five (25.1%) of the women had a trouble when they chose the hospital. 2. At least 90% of the surveyed women recognized the role of a midwife, although the role was considered to be an indirect one by the majority. A high proportion of the women recognized that the midwife's role was valuable with regard to "Birth" and "Care after childbirth", whereas only a low proportion recognized a midwife's role in diagnosis. 3. With regard to expectations of a midwife, some items during the pregnancy period were considered to have been enhanced by the midwife's care. A large proportion of women requested the need for set standards of care at the time of delivery. After childbirth, many aspects of after-care after discharge from hospital were considered to require midwife support.
