顎裂を有する患者に対して,顎裂に近接する中切歯(以下,中切歯と略)萌出前に顎裂への二次的骨移植術(以下,骨移植と略)を行った。本研究の目的は,中切歯萌出前における骨移植後の中切歯に対する矯正治療について調査し,中切歯萌出前における骨移植の利点の検討を行うことである。対象症例は東北大学病院顎口腔機能治療部にて咬合管理を行っている患者のうち,1997年2月から2003年12月までの期間に,東北大学病院形成外科にて中切歯萌出前における骨移植が施行された片側性口唇顎裂および片側性口唇口蓋裂患者46例である。これら患者の中切歯萌出後の矯正治療の有無を調べた。その結果,全46例中31例に切歯部に矯正治療が適用されていた。中切歯萌出前における骨移植後に萌出した中切歯に対しては多くの症例で矯正治療が必要であった。しかし,既に顎裂骨欠損が骨移植により修復されていることから,顎裂のない症例と同様に顎裂に近接する歯の矯正治療が可能であった。このような中切歯萌出前における骨移植の利点は,骨移植時期の検討に際して新たな視座を与えると考えられた。This study reviews our experience with secondary alveolar bone grafting (ABG) before eruption of the cleft-adjacent central incisors followed by orthodontic treatment of the cleft-adjacent central incisors when necessary. Forty-six patients with unilateral cleft lip and alveolus or cleft lip and palate were evaluated. They had received ABG before eruption of the cleft-adjacent central incisors between February 1997 and December 2003 at the Department of Plastic and Reconstructive Surgery, Tohoku University Hospital. All orthodontic treatments were performed at the Clinics for Maxillo-Oral Disorders, Tohoku University Hospital. Thirty-one of the 46 patients had completed alignment of the cleft-adjacent central incisors, which showed severe inclination/rotation. Thus, two thirds of the patients (31/46) required alignment of their cleft-adjacent central incisors. ABG before eruption of the cleft-adjacent central incisors facilitated subsequent orthodontic alignment of the incisors, because the procedure provided a foundation that would otherwise have been impaired by the alveolar cleft.
