We participated in the development of clinical guidelines for the management of cleft lip and palate, coordinated by the Japanese Society of Plastic and Reconstructive Surgery. In this paper, we present six clinical questions (CQs) on the management of residual velopharyngeal insufficiency following primary palatal closure. We also include comments and suggestions regarding questionable or difficult issues raised during the process of creating these CQs.
